Why conventional ranges miss diagnoses
The Traditional Problem
The traditional "normal" range is often too broad because it includes older, undiagnosed individuals and those with hidden autoimmune conditions. This legacy approach misses early-stage hypothyroidism and occult thyroid dysfunction.
The New Normal
Evidence indicates that the optimal upper limit for TSH is generally 2.5 mIU/L. Readings between 2.5 and 4.0 mIU/L may serve as early warning signs of hypothyroidism, warranting further investigation and intervention.
Autoimmunity & TSH
Up to 30% of individuals with a TSH above 3.0 mIU/L test positive for antithyroid antibodies, indicating Hashimoto's Disease. This hidden autoimmunity often goes undiagnosed in conventional screening.

Fluoride & Thyroid Function
Over the past decade, several studies have focused on the effects of environmental toxins on the human endocrine system, including the impact of fluoride on the thyroid gland. A systematic review examining fluoride exposure and thyroid function across 190 peer-reviewed studies from 2007–2016 documented a consistent relationship between fluoride ingestion and altered thyroid hormone metabolism, as well as associations with dental fluorosis and thyroid disease.
Ruiz-Payan et al. (2007) found that even at 1 mg/L of fluoride in water, T3 levels were significantly reduced in teenagers. Basha et al. (2011) found fluoride reduces T4 and T3 levels with generational and cumulative effects on offspring development. Tang et al. (2018) demonstrated dose-dependent suppression of thyroid peroxidase (TPO) activity in vitro.
Clinical Recommendation
Reverse-osmosis and activated-carbon water filtration is recommended for patients with hypothyroidism or Hashimoto's thyroiditis, particularly in fluoridated municipalities. Targeted fluoride reduction may improve TSH levels and reduce thyroid antibody titers in predisposed individuals (King et al., 2020).
Dietary Factors
Gluten & Autoimmune Thyroid Disorders
Celiac disease is often associated with various autoimmune conditions, including autoimmune thyroid disorders (AITDs). The prevalence of IgG and IgA anti-tissue transglutaminase antibodies and IgA anti-gliadin antibody was higher in Hashimoto's thyroiditis patients compared with the control group. Serum IgG anti-tissue transglutaminase and IgA anti-gliadin antibodies were significant predictors of antithyroid antibodies in patients with Hashimoto's thyroiditis (P < 0.05), and a significant relationship between serum TSH and IgG anti-gliadin antibody was also found (P = 0.003).
Clinical Conclusion
Strict adherence to a gluten-free diet may be beneficial for individuals with both celiac disease and AITDs. A deeper understanding of the gut–thyroid axis could lead to innovative approaches in the management of autoimmune thyroid disorders.
Microbiota–Gut–Thyroid Axis
Iodine & Hashimoto's Thyroiditis
Excess iodine induces hypothyroidism in Hashimoto's thyroiditis. Emerging evidence highlights the gut microbiota's role in HT pathogenesis via the microbiota–gut–thyroid axis. This study examined how iodine affects gut dysbiosis and HT, recruiting 23 patients with HT and 25 healthy individuals. Butanoate metabolism significantly differs between groups, and butyric acid is significantly decreased in patients with HT.
Gut dysbiosis, driven by excessive iodine intake, disrupts the TH17/Treg balance by reducing butyric acid. Iodine intake alters intestinal microbiota composition and metabolic changes, influencing the microbiota–gut–thyroid axis. Sodium butyrate significantly ameliorated intestinal epithelial barrier dysfunction and reshaped the gut microbiota in the HT mouse model.
